2	Std II to Std IV : The School leaving certificate from the last school attended. Progress report of the last qualifying examination. If the student has come on a transfer from another state or district, the school leaving certificate must be countersigned by the concerned District Education Officer.

1	Passport size latest photograph.
2	Std. V – IX Original school living certificate from last school attended.
3	Progress report of the last qualifying examination.
